Question
I refer to page 2 of the Voluntary Assisted Dying Board Western Australia – Annual Report 2021-22 which states that ‘Data was extracted from VAD-IMS on 5 October 2022 to account for activity that occurred prior to 30 June 2022 … Unless specified otherwise, data in the annual report reflects information collected in valid forms only. VAD-IMS also holds data on forms with other status types including void, revoked or invalid’, and I ask:
(a) how many valid forms were relied upon in
preparation of the Voluntary Assisted Dying Board Western Australia Annual
Report 2021-22;
(b) for what reason(s) might a form be found to be
void;
(c) how many forms were found to be void for the
reporting period;
(d) of those forms found to be void for the
reporting period, were any of the patients identified in these void forms
supplied with voluntary assisted dying substances;
(e) for what reason(s) might a form be revoked;
(f) how many forms were found to have been revoked
for the reporting period;
(g) of those forms found to have been revoked for
the reporting period, were any of the patients in these revoked forms supplied
with voluntary assisted dying substances;
(h) for what reason(s) might a form be found to be
invalid;
(i) how many forms were found to be invalid for the
reporting period; and
(j) of these forms found to be invalid for the
reporting period, were any of the patients
in these invalid forms supplied with voluntary assisted dying
substances?

(b) As stated on page 27 of the Annual Report: ‘A form is assigned a status of ‘void’ when a subsequent Consulting Assessment Form is submitted, or a form has been superseded by another valid submission.’

(d) Yes. For a voluntary assisted dying substance to be supplied, a valid form must be submitted at each proceeding stage of the process. Forms which were void on submission were superseded by valid forms and voluntary assisted dying substances were supplied.
(e) As stated on page 27 of the Annual Report: a form is assigned a status of ‘revoked’ when a patient has revoked their administration decision or appointment of a Contact Person.’

(g) Yes. For a voluntary assisted dying substance to be supplied, a valid form must be submitted at each proceeding stage of the process. Forms which were revoked following submission were subsequently replaced with valid forms and voluntary assisted dying substances were supplied.
(h) Reasons a form can be deemed invalid include the submission of an incomplete form, a submission error, a duplicate submission, or the form contains invalid information.
